Safety Alert
A new worker performed a verified lockout in an MCC room. A cable was used to lockout the main and the 3 separate disconnects.When the verifier performed the test with the worker, the in-feed chains which was one of the separate disconnects started up.
The disconnect was tagged and when the worker locked out the local disconnect, outside the MCC room, for the equipment, the test passed. The equipment did not start up.
